On Christmas Eve, a man drives his wife Sarah and their five-year-old daughter Lily back to his Ohio hometown. The gathering turns cold when his Aunt Carol pulls him aside and demands he hand over Lily's guardianship to her son, Jack, claiming a psychic told her his next child will be a boy. He refuses, and the night collapses into fury and violence.

He discovers Jack's phone holds a folder named "Angel" filled with pictures of Lily, and the family turns on him instead of Jack. Fleeing to a motel, they learn Jack works at an auto repair shop. The next morning, as they drive away, the rental car's brakes fail on the icy road and the car plunges through the guardrail.

He wakes in a medical center, the family's nightmare far from over.
