When her mother-in-law, Amanda White, collapses from a heart attack, Lorraine rushes her to the hospital, trusting her husband Eugene Shaw, a celebrated cardiac surgeon, to save her. Instead, Eugene hands the operation to Peggy Flannery, an intern of only three days, who is practicing carving words into a patient's heart. Her carelessness punctures a valve, and Amanda dies on the table with two letters, PF, cut into her heart.

Eugene declares the surgery a failure and tells Lorraine to prepare the funeral, then demands she sign a letter of forgiveness so Peggy is never blamed. He slaps her when she asks to see the surgical footage, insisting her mother was simply old and frail. But the name on the document reads Amanda White, and the body beneath the sheet is his own mother, not Lorraine's.

Lorraine refuses to sign, and when Eugene threatens divorce, she hands him the papers she has already drawn up. He still believes the dead woman is her mother, and she waits for the moment he learns the truth.
