Ethan is an on-call doctor in a busy Emergency Room, accustomed to strange cases arriving at night. One night, the patient wheeled in is his own wife, Savannah, unconscious and pale. Examining her, he discovers a rectal tear caused by a foreign object, a wound that tells him she has been with someone else. Betrayal and humiliation crash over him as he operates to remove the object and stitch her injury, all while the night nurse, Amber, watches with what feels to him like judgment.

When Savannah wakes, she insists it is a misunderstanding and begs to explain, but Ethan refuses to listen and walks out. At home he drinks himself into a stupor until his best friend, Landon, arrives, sent by Savannah. Landon warns him that without hard proof of cheating, a divorce would cost him half his house, so Ethan resolves to wait and gather evidence.

Two days later Savannah is discharged and comes to the ER, holding out her phone. She claims a video will explain everything. Ethan snatches it, and her scream fills the room.
