A woman wakes after minor surgery to find herself locked in a psychiatric hospital, where an intercom announces a deadly game: forty patients, hidden impostors among them, and a daily public vote that ends in execution for whoever is chosen. If every impostor is found within five days, the patients survive; if any remain, all are wiped out. Medication time brings its own trap — blue pills are safe, red pills kill — and she is handed a red one.

Surrounded by violent and unpredictable patients, she fakes swallowing the poison and spits it out in the restroom, only to be confronted by a young man who reveals he is an impostor too. He presses her to slip her red pill into the food of a man in his fifties who has been hunting impostors, and she does it. When the vote comes, a little girl points at the young man, exposing that the dead man's target was never her — she was only a pawn.

Now the votes are being counted, and the woman must decide who to trust in a place where killing is allowed and every ally may be a lie.
