Milena's husband Steven is a celebrated heart surgeon, but when their twins die during surgery, he leaves her clutching their ashes and tells her the children were destined to die. That night, his intern anesthesiologist Kelly Johansson posts photos of Disney fireworks and a man whose scar matches Steven's, hinting at an affair. Milena replies that the two should be together, and Steven threatens divorce unless she deletes it.

Milena owns the private hospital that made Steven its chief physician. She adds him and Kelly to the list of staff assigned to the African branch for major medical malpractice, and fires the HR manager who pleads for them. Steven calls in a rage, insisting the children's deaths were God's will, while Kelly's voice is heard asking him to watch the fireworks.

Holding the children's urns, Milena vows never to let them go. The next morning, Steven serves her porridge with a smile.
